If you’re considering becoming a donor, you must start by filling out our self-evaluation form named BREEZE. After you’ve finished filling out the questionnaire, you will be contacted by our Living Donor Nurse Coordinator, who will set up your first appointment at Lahey Hospital to begin learning about the transplant process.

Self-evaluation

Your first step will be to fill out the Breeze self-evaluation questionnaire. This is a simple online form that will ask some basic questions about your medical history and physical condition. It will help us determine whether you're a viable candidate, so all living donors need to fill it out before being considered for organ donation. Click here to begin.

Your First Visit

After finishing the Breeze questionnaire, you'll be contacted by our nurse coordinator to set up an appointment. Due to the moral and legal implications of organ donation, UNOS mandates that you should visit us individually, without your intended recipient being present, as he or she might unintentionally influence your decision. You are also encouraged to talk to the donor team with any concerns, and take as much time as you need to think your decision through. Your decision to become a donor should be stress-free.

Donor Confidentiality

Lahey Hospital will collect and generate personal health and social information throughout your donation process. This information will remain confidential, even from the person you’re donating to. If third parties wish to obtain any of your information (UNOS for example) we’ll share it only if we have your authorization to do so.

Opting Out

You might learn aspects of kidney donation that you didn’t take into consideration when you first volunteered (risks of surgery, for example). We try to be as forthcoming as possible to make you aware of all implications, whether good or bad. If you change your mind and no longer wish to donate, you may opt-out without consequences. The reasons behind your decision will remain confidential. This can be done at any point up to the moment of surgery.
